ABOUT CASE MANAGEMENT

Compass’ case management department provides medical case management services that promote healthy outcomes for people living with HIV/AIDS (PLWHAs).  We accomplish this by assessing our clients’ medical needs, developing a plan of care that addresses those needs, and identifying barriers i.e., lack of transportation, legal issues, food, or housing, that may hinder their ability to access medical care. 

Compass case managers are cross-trained to provide case management services in the local health departments.  Compass is currently the sole contract provider with the Palm Beach County Health Department to provide onsite case management services in a high-volume medical setting.

Through case management services, clients are empowered to decrease isolation, create social/support networks, enhance their quality of life, and achieve positive outcomes according to their individual needs.
