Case Management

Compass Center

Compass’ Health Services Department provides medical and non-medical case management as well as mental health and peer support services that promote healthy outcomes for people living with HIV/AIDS (PLWHAs).  We accomplish this by assessing our clients’ medical and psychosocial needs and identifying barriers to medical care such as access to health insurance, lack of transportation, legal issues, food, or housing. In collaboration with the health services’ team, the client develops a plan of care that addresses his or her needs. Additionally, we provide social/support groups and community education that addresses the physical, mental, and social needs of people living with HIV/AIDS. Through health services, clients are empowered to become self-sufficient, decrease isolation, create social and support networks, enhance their quality of life, and achieve positive outcomes according to their individual needs.

During the case management intake process, clients are screened for mental health needs. Clients can be referred for individual and group mental health services. Compass has an on-site Licensed Mental Health Therapist to provide group and individual counseling sessions to individuals living with HIV/AIDS.  Please contact our Licensed Mental Health Therapist at 561-533-9699 Ext 4055 for more information.
